External evaluation of Closing the Gap, Florida Department of Health’s grant program to reduce racial and ethnic health disparities
This scope of work is to perform evaluation services for the Reducing Racial and Ethnic Health Disparities “Closing the Gap” (CTG) Grant program. The services include documenting historical efforts, evaluating current efforts, developing a new program reporting structure, and providing technical assistance to grantees and the Department. Contractor will provide these services to the Florida Department of Health (Department).
